This permanent role blends hospital-based clinical pharmacy with patient-facing ambulatory care and chronic disease management. Terre Haute offers a low cost of living, manageable commute times, and easy access to Indianapolis (approximately one hour away). The community is home to Indiana State University and Rose-Hulman Institute of Technology, contributing to a stable local economy and a welcoming, close-knit feel. Schedule • Full-time, 40 hours/week • Day shift, varied daytime schedule Compensation & Incentives • Hourly range: $53.37 – $80.06 • Annual range: ~$111,009 – ~$166,524 (DOE) • $20,000 sign-on bonus (2-year commitment) • Full benefits package Role Highlights • Hospital-based role with strong ambulatory and outpatient integration • Involvement in chronic disease and anticoagulation management • Participation in interdisciplinary clinical committees • Stable day-shift schedule with long-term stability • Opportunities for professional development and specialization Key Responsibilities • Perform drug information and dosing consultations • Participate in clinical committees (P&T, Sepsis, Antimicrobial Stewardship, Medication Safety) • Support anticoagulation management and collaborative drug therapy management (CDTM) protocols • Provide patient and staff education • Maintain documentation in EPIC Requirements • PharmD from an accredited program • Licensed or eligible for Indiana licensure • Residency or equivalent clinical experience preferred • Ambulatory experience preferred • Illinois licensure a plus Medical Solutions Direct Hire Job ID 1043824.
