POSITION SUMMARY
- The incumbent shall be responsible for providing nursing
services including assessment, treatment planning and therapeutic
interventions in an interdisciplinary environment consistent with
the position's qualifications, professional practices and ethical
standards. the incumbent shall also be accountable for and
contribute to program development, quality improvement, problem
solving and productivity enhancement in a flexible
interdisciplinary fashion.JOB DUTIES AND RESPONSIBILITIES

- Coordinate the development of team members through structured
activities
- Nursing staff orientation is planned and conducted in
coordination with hospital nursing services policies
- Assist program director in identifying education needs of
nursing staff and implement in-service sessions
- Attend/present at unit in-service sessions as scheduled
- Encourage and assist with the development of student
programs
- Assist program director/hospital nursing services with
recruitment and interviewing process
- Nursing staff is supervised and input is provided to hospital
regarding evaluations as requested.
- Function as a member of the leadership team with the program
director and the medical director
- Attend meetings and actively participates in problem
identification and the development of options for resolution
- Assist with budget preparation
- Promote the integration of the rehabilitation of the unit into
hospital culture
- Horizontal and vertical flow of positive communication is
maintained with all departments of the hospital
- participate in hospital committees as directed by program
director or requested by hospital
- Communicate and demonstrate a professional image/attitude for
patients, families, clients, coworkers and others
- Adhere to applicable dress code
- Adjust personal schedule to meet needs of the unit when
requested
- Protect patient confidentiality at all times and facilitate
effective communication with all patients
- Communicate professionally with various publics as evidenced by
customer/client satisfaction
- Promote adherence to policies and practices of applicable
professional organizations, client hospital and RehabCare Group
- Is familiar with and acts as a resource for team members
regarding the contents of applicable professional code, hospital
policies, unit p&P, and company SOP/handbook
- Contribute to client relations and marketing activities
- Assist the program director in establishing contact with
discharge planners, physicians and other referral sources
- Establish a positive relationship with facility personnel
including administration nursing, and therapy team members
- Monitor nursing staff competency, licenses/certifications and
memberships in applicable professional organizations
- A competency and/or skills assessment is satisfactorily
completed annually by all
- Applicable licenses/certifications are current for all clinical
team members
- Incumbent license/certifications and memberships are
current.
- Maintain clinical expertise in the field of rehabilitation, for
the age population(s) served, through practice and continuing
education
- Assume the responsibilities of team leader/charge nurse as
necessary
- attend external professional growth activities as scheduled and
communicate information to the team
- Participate in pre-admission screening
- Pre-admission is completed according to policy and
procedure
- Conduct initial assessments
- Choose the appropriate assessment tools, adaptive equipment and
material/props used in the assessment based on the age and
developmental level of the patient
- Patient assessment is completed and in the medical record
within eight hours after admission. Data is recorded in the
appropriate areas
- Document the source of patient care including progress made and
continuing need for treatment
- Weekly progress notes are completed according to policy and
procedure
- Discharge summaries are completed according to policy and
procedure
- Provide patient and family education on tasks and resources
that will assist with continued functional independence.
- Integrate family members into the treatment, educational and
discharge planning process for a safe transition home, and to
ensure continued care needs are met
- Effectiveness of education measured by peer input, family
verbal feedback and patient satisfaction surveys
- Favorable attitudes and motivation toward the rehabilitation
process are evidenced by positive patient satisfaction surveys
- Act as a resources to team members in the development of new
methods of delivering education to patients an family
- Gather input from team members on the needs of patients and
families for education
- Ensure that educational methods are adapted to meet patient and
family needs and are based on patient age, cognitive abilities
and/or developmental level
- Coordinate treatment activity between nursing and therapy team
members
- Facilities nursing involvement in the interdisciplinary team
process
- Participate in the development of interdisciplinary program
components (i.e. ADL routines, bowel/bladder training).
- Participate in clinical staffing and encourage team members to
provide/seek input to/from others on patient care
- Solicit input from nursing/therapy team members on issues
relating to patient care, team development and program
development
- Use and update unit communication methods in a timely
manner
- Take immediate action through appropriate channels to correct
adverse situations involving team members
- Participate in team business meetings
- Attend team business meetings and offer ideas/ solutions in
problem solving situations/activities regarding the problem
- Contribute to development of improved efficiency and
productivity of unit clinical/administrative functions
- Work with program director to develop, institute, revise and
promote plans for effectively achieving unit goals
- Facilitate the implementation of system improvements, and new
program components
- Offer to assist other team members and demonstrates flexibility
in carrying out shared team functions including special
projects
- Participate in coordination of unit quality management and
program evaluation
- attend/lead scheduled unity quality management meetings as
assigned
- Develop, initiate and maintain the CQI plan
- Integrate the nursing CQI activities into the hospital
plan
- assist team members in the interpretation of reports/data to
improve quality of care delivered
- Participate in the formulation of rehabilitation nursing policy
- Review policies and procedures on an annual basis for continues
appropriateness.
- Make revisions as appropriate to maintain compliance with
applicable licensing and regulatory requirements and practice
developments
- Participate in survey preparation activities
- demonstrate knowledge of criteria/accredidation standards and
assist with survey preparation as assigned
- Interact with assigned program consultant
- Participate as requested in program consultant site visits and
follow through on requested tasks
- Work with the program consultant to effectively implement
program components identified in company P&P
- Participate in unit safety committee activities
- Attend meetings and complete assigned duties as requested
- Promote compliance with safety standards and reduction of
potential hazards
- Evaluate ongoing data as it pertains to fall rates and other
significant incidences
- Communicate finding and activities of the safety committee with
staff
- Contribute to the delivery of cost effective care
- Maximize nursing staff productivity through effective
scheduling
- Utilize resources appropriately and use/maintain equipment in
order to prolong working life.

- Level of Education

- Minimum: Graduate of a nationally approved school of
nursing
- Preferred: BSN/ CRRN
- Level of Experience

- Minimum: Experience working with an adult geriatric
population
- Preferred: Experience serving people with physical
